Did you know that if your aged 40 and over you are entitled to a free NHS health check? Just call the surgery and ask reception to book you an appointment, please note this does require a fasting blood test so it will be a morning appointment

The surgery telephone lines are open from 8am to 6.30pm Monday to Friday, except for Public Holidays... If you require urgent medical assistance that cannot wait until the surgery re-opens, please call 1-1-1. Calls to the NHS 111 service are free from both landlines and mobiles.

Welcome to Chadderton Medical Practice Online

Due to the current outbreak of coronavirus we have followed national guidelines and we are taking precautionary measures to minimise the number of patients attending the surgery. So we have made the following changes: we have temporarily suspended online appointment bookings. And appointments will all be telephone triages first. If you are asked to attend the surgery you must be wearing a face covering or mask and use the hand sanitiser provided. You will also have your temperature taken before being allowed to enter.  While waiting to enter the building from the back door, you must adhere to the 2 metre distancing rule and wait in a safe place away from moving traffic.

ATTENTION!!!
The doors into the waiting room are now going to be locked. If you have an appointment you must call us once your at the surgery on 0161 909 8144 and wait in the foyer and someone will inform you of the procedure regarding entering the building. Please only attend 1 person per appointment UNLESS you a bringing a child or your a care giver and then only 1 parent or carer must attend. Face coverings must be worn and you must use the hand sanitiser provided. Your temperature will be taken before you can enter. Prescriptions MUST be ordered via prescription voicemail (0161 909 8144 then listen for the option) or via email at prescriptions.chadhealth@nhs.net DO NOT attend the surgery to drop off a prescription request. All prescription collections must go to a nominated pharmacy do not come to the surgery to collect a prescription. Perspex screens have now been placed in the reception area for our safety and yours. There will also be 2 metre markings in the waiting room.

Named Accountable GP

All patients are allocated a named, accountable GP. The named accountable GP takes responsibility for the co-ordination of all appropriate services and ensures they are delivered where required (based on the named GP's clinical judgement) but does not take on vicarious responsibility for the work of other doctors or health professionals.

 

If you wish to be told the name of your accountable GP, please ask the receptionist when you are next in the surgery. To keep the lines free for patients needing treatment, kindly do not telephone the practice to ask for this information only.

 

Having a named GP does not prevent you from seeing any other doctor in the practice, nor does it mean that you will always see your named GP, who will not be available at all times. Patients are entitled to choose to see any GP or nurse in the practice but if your needs are urgent, you may need to discuss them with an alternative doctor or nurse.
